This full-time, 13-week contract assignment offers an engaging schedule and the opportunity to make a significant impact on patient outcomes through focused, one-on-one interactions.

Schedule and Hours:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based, with daily travel to patient homes within an assigned territory
• Caseload: 30 points per week (typically 35 patient visits per day)

Key Responsibilities:

• Complete thorough OASIS assessments and develop individualized patient care plans
• Deliver skilled nursing interventions, such as wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient conditions, document progress, and communicate status changes to physicians and care team members
• Educate patients and families about their condition, treatment plans, medications, and preventive health practices
• Accurately record all care activities and updates in Home Care Home Base (HCHB)
• Collaborate closely with interdisciplinary team members, including therapists, social workers, and aides, to ensure continuity of care
• Engage in case conferences, team meetings, and agency-required education
• Uphold all documentation, ethical, and regulatory standards

Qualifications:

• Active, unrestricted RN license in the state
• Minimum 1 year of recent home health nursing experience, preferably field-based
• Advanced proficiency in Home Care Home Base (HCHB) and OASIS assessments
• Reliable transportation, valid drivers license, and auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
