Embark on a rewarding opportunity providing skilled nursing care in a dynamic home health environment. This full-time, 13-week contract role focuses on delivering high-quality patient care within a supportive interdisciplinary team.

Schedule:

• Monday to Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active, unrestricted RN license in the state of practice
• At least 1 year of recent home health nursing experience (field experience strongly preferred)
• Proficient with Home Care Home Base (HCHB) electronic documentation
• Demonstrated expertise completing OASIS assessments
• Reliable transportation, valid drivers license, and current auto insurance

Primary Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized plans of care
• Deliver skilled nursing services such as wound care, IV administration, medication management, and chronic disease education
• Regularly evaluate and document patient progress in HCHB, reporting any changes in condition promptly
• Educate patients and their families regarding disease process, medication regimes, and health maintenance
• Collaborate with multidisciplinary team membersincluding PTs, OTs, medical social workers, and home health aidesto ensure coordinated, patient-centered care
• Participate in case conferences, staff meetings, and ongoing education to support professional growth
• Maintain strict adherence to regulatory standards and agency protocols

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
