Your next nursing opportunity awaits as a Registered Nurse specializing in Home Health care. This full-time, 13-week contract role is ideal for an RN with solid clinical experience, proven OASIS skills, and a passion for delivering patient-centered care in a home setting.
Schedule:
- Full-time, Monday through Friday
- On-call availability may be required
Key Responsibilities:
- Deliver skilled in-home nursing care and interventions
- Conduct thorough patient assessments and complete OASIS documentation promptly and accurately
- Coordinate with physicians and interdisciplinary teams to implement individualized care plans
- Provide wound care, medication administration, patient education, and monitor patient progress
- Maintain clear communication regarding patient condition updates with the clinical team
Required Qualifications:
- Active Registered Nurse (RN) license
- Minimum of one year recent experience as an RN in Home Health
- Required proficiency in OASIS documentation
- Experience with HomeCare HomeBase is strongly preferred
- Ability to independently manage daily patient visits and full-time caseload
- Reliable transportation and valid driver's license
