Your next nursing opportunity awaits. An experienced Registered Nurse is needed for a full-time, 13-week home health contract, delivering comprehensive care to patients in their homes. This position is ideal for clinically skilled RNs with a strong background in OASIS documentation and a commitment to making a meaningful impact in the community.

Schedule:

• 13-week contract, with potential extension
• Monday through Friday (full-time, on-call probable)
• Structured daily visit expectations

Core Responsibilities:

• Deliver skilled in-home nursing care, including assessments and interventions
• Complete OASIS documentation accurately and promptly for each patient
• Collaborate closely with physicians and interdisciplinary teams to develop and execute individualized care plans
• Perform wound care, medication administration, patient education, and chronic disease monitoring
• Communicate relevant updates and changes in patient conditions to the clinical team

Qualifications:

• Active, unrestricted RN license
• Minimum 1 year of recent home health RN experience
• Demonstrated proficiency with OASIS documentation
• Strongly preferred: Experience with HomeCare HomeBase software
• Proven ability to manage daily home visits and maintain organized scheduling
• Valid drivers license and reliable transportation
