Exciting opportunity available for an experienced RN to provide compassionate care in a field-based home health setting. This full-time, 13-week contract focuses on supporting patients in their own homes, ensuring high-quality care, and promoting patient independence.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Caseload of approximately 35 patient visits per day (targeting 30 points per week)

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop personalized care plans for each patient.
• Deliver skilled nursing interventions, including wound care, medication administration, IV therapy, and management of chronic conditions.
• Monitor patient progress, document findings, and promptly report any changes to the healthcare team.
• Provide education and support to patients and their families regarding disease management, medications, and prevention strategies.
• Accurately document all patient care using Home Care Home Base (HCHB).
• Collaborate with interdisciplinary team members, including therapists and home health aides, for holistic patient care.
• Attend case conferences, participate in staff meetings, and engage in ongoing education, as required.
• Ensure all activities meet regulatory and agency standards for patient safety and quality.

Qualifications:

• Active RN License in the state of practice, in good standing.
• At least 1 year of recent home health field nursing experience strongly preferred.
• Demonstrated experience with OASIS and proficiency in Home Care Home Base (HCHB) system.
• Reliable transportation, current drivers license, and proof of auto insurance.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
