Job Description

Deliver high-quality skilled nursing care as a Home Health Registered Nurse (RN) in a field-based role, providing essential services to patients in their homes. This full-time, 13-week contract position requires clinical expertise, strong communication skills, and the ability to work independently within an assigned territory.

Location:
The position is based in McKenzie, AL, a close-knit community offering a peaceful setting with access to local amenities and a welcoming atmosphere for healthcare professionals seeking a field nursing role.

Schedule:

  • Monday Friday
  • 8:00 AM 5:00 PM

Key Responsibilities:

  • Conduct comprehensive OASIS assessments and develop individualized care plans tailored to patient needs.
  • Deliver skilled nursing care, including wound care, medication administration, IV therapy, and management of chronic diseases.
  • Monitor patient progress, detect changes in conditions early, and communicate effectively with interdisciplinary teams and physicians.
  • Educate patients and families on health conditions, treatment protocols, and preventive care measures.
  • Maintain accurate, timely documentation in Home Care Home Base (HCHB).
  • Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides to ensure seamless, coordinated care.
  • Participate in case conferences, staff meetings, and ongoing professional education.
  • Comply with all regulatory and agency standards related to care delivery and documentation.

Qualifications:

  • Active RN license in good standing for the state of practice.
  • Minimum 1 year of recent home health nursing experience, preferably in field-based care.
  • Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools required.
  • Strong clinical assessment and critical thinking abilities.
  • Reliable transportation, valid drivers license, and auto insurance for daily travel to patient homes.
  • Ability to work independently with excellent time management and organizational skills.

Preferred Experience:

  • Bachelor of Science in Nursing (BSN) preferred.
  • Experience managing 2532 points per week caseload.
  • Familiarity with Medicare regulations and home health compliance standards.
