Schedule:
- 13-week contract
- Full-time hours, Monday through Friday
- On-call rotation likely
Key Responsibilities:
- Deliver in-home skilled nursing care and interventions for a variety of patient needs
- Complete thorough patient assessments and OASIS documentation accurately and on time
- Collaborate with physicians and multidisciplinary care teams to develop individualized care plans
- Administer wound care, manage medications, provide patient education, and monitor chronic conditions
- Communicate changes in patient condition promptly to the clinical team
- Ensure all visits, records, and communications align with regulatory standards
Qualifications:
- Active RN license
- Minimum one year of recent home health RN experience
- Demonstrated OASIS documentation experience
- Proficiency with HomeCare HomeBase (strongly preferred)
- Organized and comfortable managing daily visit schedules
- Reliable transportation and valid drivers license
