Your next nursing opportunity awaits: A rewarding 13-week full-time contract is now open for a skilled Registered Nurse with proven home health and OASIS experience. If you excel at accurate documentation, independent patient care, and managing a structured schedule, this role offers the chance to make a significant impact in patients lives.
Schedule:
- Full-time, Monday through Friday
- On-call likely included
- Daily visit structure with clear expectations
Key Responsibilities:
- Deliver skilled nursing care, interventions, and ongoing patient education during in-home visits
- Conduct comprehensive assessments and complete OASIS documentation with attention to accuracy and timeliness
- Implement individualized patient care plans in collaboration with physicians and interdisciplinary teams
- Provide wound care, medication management, and disease monitoring as needed
- Proactively communicate changes in patient conditions to the clinical team
Qualifications:
- Active RN license in good standing
- At least 1 year of recent home health RN experience
- Demonstrated experience with OASIS documentation
- Familiarity with HomeCare HomeBase (preferred)
- Exceptional clinical judgment, organization, and time management skills
- Dependable transportation and valid drivers license
