We are seeking a dedicated Registered Nurse (RN) with strong home health experience and proficiency in OASIS documentation for a 13-week full-time contract. Schedule:
- Full-time, Monday through Friday
- On-call participation may be required
- Daily patient visit structure ensures consistency and clear expectations
Key Responsibilities:
- Deliver comprehensive in-home nursing care, including wound care, disease monitoring, and medication management
- Complete detailed patient assessments and OASIS documentation promptly and accurately
- Collaborate with physicians and interdisciplinary care teams to develop, implement, and adjust individualized care plans
- Communicate changes in patient condition and progression to the clinical team in a timely manner
- Educate patients and families on disease management and self-care strategies
Qualifications:
- Current and active RN license
- Minimum of 1 year of recent home health RN experience
- Demonstrated expertise with OASIS documentation
- Experience with HomeCare HomeBase (strongly preferred)
- Valid drivers license and reliable transportation
- Ability to effectively manage full-time, structured patient visit schedules
