A highly skilled registered nurse is needed to provide expert care in a home health setting for a full-time, 13-week contract assignment. This field-based role centers around direct patient visits, delivering critical nursing interventions while making a real impact in your community.

Schedule & Assignment Details:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• 30 points per week caseload (typically 35 patient visits per day)
• Fieldwork with daily travel to patient homes within an assigned territory (mileage reimbursement provided)

Qualifications:

• Active RN license (in good standing) in the state of practice
• Minimum 1 year of recent experience in adult home health nursing (field experience strongly preferred)
• Demonstrated proficiency with Home Care Home Base (HCHB) electronic documentation system
• Experience performing OASIS assessments
• Reliable transportation, valid drivers license, and auto insurance

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop personalized care plans
• Deliver skilled nursing care including wound treatment, medication administration, IV therapy, and chronic disease education
• Monitor patient progress, promptly report changes, and communicate with the care team and physicians
• Provide thorough education to patients and families regarding disease processes, medications, and care plans
• Collaborate with physical therapists, occupational therapists, social workers, and home health aides for integrated patient care
• Accurately maintain documentation using HCHB
• Attend required case conferences, staff meetings, and participate in ongoing education
• Ensure care delivery and documentation align with all regulatory and agency standards

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
