A rewarding opportunity is available for a Registered Nurse skilled in home health care to provide patient-centered services in a field-based environment. This full-time, 13-week contract role offers a consistent weekday schedule and the chance to make a meaningful impact on patients lives through personalized care and education.

Schedule and Key Details:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based position visiting patient homes
• Caseload of 30 points per week (typically 35 patient visits per day)
• Utilizes Home Care Home Base (HCHB) electronic documentation
• OASIS assessment tool experience required
• Daily travel within an assigned territory (mileage reimbursement provided)

Preferred Qualifications:

• Active RN license in good standing in the state of practice
• Minimum one year of recent home health experience (field experience highly preferred)
• Strong proficiency in Home Care Home Base (HCHB) documentation
• Demonstrated OASIS assessment expertise
• Reliable vehicle, valid drivers license, and current auto insurance

Primary Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans for home health patients
• Deliver skilled nursing including wound care, medication management, IV therapy, and support for chronic conditions
• Evaluate patient progress, identify condition changes, and collaborate with physicians and team members as needed
• Provide patient and family education regarding care plans, medications, and health maintenance
• Complete accurate and timely documentation in HCHB
• Maintain close coordination with interdisciplinary team members and participate in meetings and continuing education
• Ensure compliance with all regulatory and agency guidelines

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
