We are seeking an experienced Registered Nurse who thrives in the home health setting, with expertise in OASIS documentation, for a full-time, 13-week contract assignment. This role is perfect for clinicians who value independent work while being part of a supportive care team and want to see the real impact they have on patients lives.
Schedule:
- Full-time, Monday through Friday
- On-call possible, as needed
- Daily visit expectations
Key Responsibilities:
- Deliver skilled in-home nursing care, including wound care, medication management, and patient education
- Perform comprehensive assessments and complete OASIS documentation accurately and timely
- Collaborate closely with physicians and interdisciplinary care teams to ensure appropriate care planning and execution
- Monitor and report patient status and changes in condition to team members and leadership
- Use your clinical judgment to manage care schedules and efficiently handle daily visits
Qualifications:
- Active RN license in good standing
- Minimum of 1 year of recent home health RN experience
- Required proficiency with OASIS documentation
- Experience with HomeCare HomeBase (highly preferred)
- Ability to handle structured schedules and multiple daily patient visits
- Reliable transportation and a valid drivers license
