Experienced registered nurses with a background in home health care are invited to join a dedicated field-based team for a full-time, 13-week assignment providing skilled nursing services directly to patients in their homes. This role is ideal for professionals who thrive working autonomously, value patient-centered care, and have strong documentation and assessment skills.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based, providing care in patient homes within an assigned area
• Expect 30 points per week, typically translating to 35 patient visits daily

Qualifications:

• Must hold an active, unrestricted RN license for the state
• Minimum of 1 year of recent home health experience is required, with fieldwork strongly preferred
• Demonstrated proficiency with Home Care Home Base (HCHB)
• OASIS experience is required
• Reliable transportation, valid drivers license, and current auto insurance

Role Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized plans of care
• Deliver skilled nursing interventions: wound care, medication management, IV therapies, and chronic disease management
• Continuously assess patient status, noting changes and updating care teams and physicians promptly
• Educate patients and families on health conditions, care plans, medications, and preventive strategies
• Ensure detailed, accurate, and timely documentation via Home Care Home Base
• Collaborate closely with the multidisciplinary care team to optimize outcomes
• Attend case conferences, meetings, and professional development opportunities
• Adhere to standards for patient care and regulatory compliance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
