Schedule:
- Full-time, Monday through Friday
- On-call availability probable
- Daily visit expectations structured for consistency
Key Responsibilities:
- Deliver compassionate in-home nursing care and skilled interventions
- Conduct comprehensive patient assessments with precise and timely OASIS documentation
- Collaborate with physicians and multidisciplinary care teams to develop individualized care plans
- Provide wound care, medication management, disease monitoring, and patient education
- Communicate patient updates and any changes in condition to the clinical team promptly
Qualifications:
- Valid RN license
- At least 1 year of recent experience as a home health RN
- Proficiency with OASIS documentation required
- Experience with HomeCare HomeBase strongly preferred
- Capable of managing a structured, full-time schedule with daily patient visits
- Reliable transportation and a current drivers license
