This position offers a focused 13-week full-time contract designed for professionals skilled in clinical nursing and OASIS documentation, seeking to deliver compassionate in-home care while managing a structured schedule.
Location: Manchester, NH
Responsibilities:
- Provide skilled nursing care and interventions within patients homes
- Perform thorough patient assessments and complete accurate OASIS documentation timely
- Collaborate with physicians and interdisciplinary care teams to develop and implement personalized care plans
- Administer wound care, medication management, patient education, and monitor disease progression
- Communicate patient condition updates promptly to the clinical team
Qualifications:
- Active Registered Nurse license
- Minimum of 1 year recent home health RN experience
- Proven expertise with OASIS documentation is mandatory
- Familiarity with HomeCare HomeBase is strongly preferred
- Reliable transportation and valid drivers license
- Ability to manage a structured full-time schedule with consistent daily patient visits
Schedule:
- Full-time, Monday through Friday
- On-call duties probable
