Deliver exceptional care to patients in a field-based setting, supporting individuals in their homes through a full-time, 13-week contract opportunity. This role is ideal for experienced Home Health RNs who excel at independent work, thorough assessments, and effective care coordination.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Expectation of 30 points per week (typically 35 patient visits per day)

Key Qualifications:

• Active RN license in the state of practice and in good standing
• At least 1 year of recent Home Health nursing experience (field-based strongly preferred)
• Demonstrated proficiency with Home Care Home Base (HCHB) for documentation
• Extensive experience with OASIS assessments and protocols
• Dependable transportation, a valid drivers license, and current auto insurance

Responsibilities:

• Complete comprehensive OASIS assessments and develop tailored care plans for each patient
• Deliver a wide range of skilled nursing services, including medication administration, wound care, IV therapies, and chronic disease management
• Monitor and document patient progress, promptly report changes to care team and physicians
• Educate patients and their families about their conditions, treatments, and preventive health strategies
• Maintain accurate, timely records within the Home Care Home Base system
• Work collaboratively with interdisciplinary team members such as therapists, social workers, and home health aides to deliver holistic care
• Attend required case conferences, team meetings, and ongoing education sessions
• Follow all regulatory and organizational policies for documentation and patient care

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
