Embark on a meaningful role delivering skilled nursing care directly to patients in a home health setting for a 13-week full-time contract. This position focuses on field-based care, supporting individuals in their homes with a schedule of Monday Friday, 8:00 AM 5:00 PM. Expect a manageable caseload of roughly 35 patient visits per day (30 points per week) and daily travel within your designated territory, with mileage reimbursement included.

Qualifications:

• Active RN license in good standing in the state of practice
• At least 1 year of recent Home Health nursing experience (field experience strongly preferred)
• Proficiency with Home Care Home Base (HCHB) electronic documentation system is required
• Solid understanding and proven use of OASIS for patient assessments
• Reliable transportation, valid drivers license, and current auto insurance

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop tailored care plans for each patient
• Deliver skilled nursing interventions including wound care, medication administration, IV therapy, and management of chronic conditions
• Monitor patient progress, recognize changes in condition, and ensure prompt communication with physicians and the care team
• Provide education to patients and families regarding disease processes, treatment plans, medication use, and preventive measures
• Maintain meticulous and timely electronic documentation using HCHB
• Collaborate closely with other healthcare professionals such as PTs, OTs, social workers, and home health aides to coordinate effective patient care
• Participate in case conferences, staff meetings, and ongoing education
• Adhere to all regulatory and agency standards for high-quality home health care

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
