Take your nursing expertise on the road by providing compassionate, skilled care to patients in their own homes through a full-time, 13-week contract focused on home health services. This field-based opportunity leverages your experience with OASIS assessments and Home Care Home Base (HCHB) documentation while delivering patient-centered care and autonomy in a supportive environment.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active, unencumbered RN license for the state of practice
• At least 1 year of recent Home Health nursing experience (field preferred)
• Proven proficiency documenting in Home Care Home Base (HCHB)
• Solid experience utilizing OASIS for patient assessments
• Valid drivers license, reliable personal transportation, and current auto insurance

Primary Responsibilities:

• Travel daily within your assigned territory, visiting 35 patients per day (30-point caseload expectation weekly)
• Conduct comprehensive OASIS assessments and develop individualized plans of care
• Deliver high-quality skilled nursing (including wound care, medication management, IV therapy, and chronic condition support)
• Assess patient progress, communicate changes, and facilitate care coordination with physicians and interdisciplinary team members
• Provide clear education to patients and their families regarding conditions, medications, and preventive care
• Maintain precise, timely clinical documentation within Home Care Home Base (HCHB)
• Participate in case conferences, team meetings, and ongoing professional development
• Uphold rigorous standards for documentation and regulatory compliance

$1,815 - $1,920/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
