An excellent opportunity is available for an experienced Registered Nurse specializing in home health. This full-time, 13-week contract role is ideal for a compassionate RN ready to provide high-quality patient care to individuals in their homes. The position is field-based, supporting patients within an assigned territory and requires daily travel, with mileage reimbursement provided.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active RN license in the state of practice, in good standing
• Minimum of one year of recent home health experience (field experience highly valued)
• Proficiency with Home Care Home Base (HCHB) electronic documentation system
• Practical knowledge and experience with OASIS assessments
• Reliable transportation, valid drivers license, and up-to-date auto insurance

Primary Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient progress, identify changes in condition, and communicate findings to the care team and physicians
• Provide patient and family education on disease processes, treatment regimens, medications, and preventive healthcare
• Maintain accurate, timely documentation within Home Care Home Base (HCHB)
• Collaborate with interdisciplinary team members, such as therapists and medical social workers, to ensure coordinated care
• Attend case conferences, staff meetings, and ongoing educational sessions as required
• Adhere to agency and regulatory standards for documentation and patient care delivery

$1,815 - $1,920/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
