A skilled and compassionate Registered Nurse (RN) with strong OASIS experience is needed for a full-time, 13-week home health contract. Professionals confident in clinical judgment, meticulous documentation, and managing a consistent schedule will find this position both rewarding and impactful.

Schedule & Duration:

  • Full-time, Monday through Friday schedule
  • 13-week contract with potential for extension
  • On-call responsibilities may be required

Key Responsibilities:

  • Deliver in-home skilled nursing care, including assessments and patient education
  • Complete OASIS documentation accurately and within specified timeframes
  • Collaborate closely with physicians and multidisciplinary teams to implement individualized care plans
  • Perform wound care, medication administration, disease management, and ongoing evaluations
  • Maintain open communication about patient status and changes to ensure the best outcomes

Desired Experience & Qualifications:

  • Active RN license in good standing
  • At least 1 year of recent RN home health experience required
  • Proficiency with OASIS documentation is essential
  • Familiarity with HomeCare HomeBase strongly preferred
  • Ability to efficiently manage a structured caseload of daily home visits
  • Reliable transportation and valid drivers license

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
