We are seeking a dedicated Registered Nurse with strong OASIS experience for a 13-week full-time home health contract. This is your chance to work in a supportive environment, managing your own patient schedule while collaborating closely with a professional care team.
Schedule:
- Full-time, Monday through Friday
- On-call availability may be required
- Structured daily visit expectations
Qualifications:
- Current and active RN license
- At least 1 year of recent experience in home health nursing
- Required expertise in OASIS documentation
- Strong preference for proficiency with HomeCare HomeBase
- Reliable transportation and a valid drivers license
- Demonstrated ability to manage a full-time structured patient schedule
Core Responsibilities:
- Deliver skilled nursing care, interventions, and patient education in home environments
- Conduct thorough assessments and complete OASIS documentation with accuracy and punctuality
- Work in close collaboration with physicians and interdisciplinary care teams to create and implement patient-centered care plans
- Provide wound care, medication management, disease monitoring, and regular patient updates
- Communicate efficiently with care teams regarding changes in patient condition
