We are seeking a skilled Registered Nurse with OASIS experience for a 13-week full-time home health contract. Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Structured daily visit expectations
Qualifications:
- Active RN license (required)
- Minimum 1 year of recent home health RN experience
- Proficiency with OASIS documentation (required)
- Experience using HomeCare HomeBase (strongly preferred)
- Ability to manage a robust, full-time schedule with daily patient visits
- Reliable transportation and valid drivers license
Responsibilities:
- Deliver in-home nursing care, performing skilled interventions per care plan
- Conduct comprehensive assessments and complete OASIS documentation with accuracy and timeliness
- Collaborate closely with physicians and interdisciplinary care teams to customize care plans
- Perform wound care, medication management, patient teaching, and disease monitoring
- Report any changes in patient condition to the broader clinical team
