A reputable team is seeking an experienced Registered Nurse with a strong background in home health for a full-time contract assignment. This 13-week role requires expertise in OASIS documentation, commitment to patient-centered care, and the ability to manage a robust visit schedule. Schedule:
- 13-week assignment with potential to extend
- Full-time, Monday through Friday
- Structured daily visit requirements and probable on-call rotation
Key Responsibilities:
- Deliver skilled in-home nursing care and clinical interventions to a diverse patient population
- Accurately conduct patient assessments and complete OASIS documentation in a timely manner
- Collaborate with physicians and interdisciplinary care teams to develop and implement individualized care plans
- Provide wound care, medication administration, patient education, and ongoing disease monitoring
- Communicate changes in patient condition effectively with the clinical team
Qualifications:
- Active RN license
- Minimum one year of recent home health RN experience
- OASIS documentation expertise required
- Familiarity with HomeCare HomeBase (strongly preferred)
- Reliable transportation and valid drivers license
- Proven ability to independently manage a structured schedule with daily home visits

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
