A dedicated Registered Nurse (RN) is sought for a 13-week full-time contract in home health, providing crucial care to patients in their residences. This assignment emphasizes efficient clinical skills, meticulous documentation, and the ability to navigate a structured schedule independently.
Schedule:
- Full-time, Monday through Friday
- On-call duties likely required
- Consistent daily visit targets
Key Responsibilities:
- Deliver skilled in-home nursing interventions with professionalism and compassion
- Conduct thorough patient assessments, completing accurate and timely OASIS documentation
- Develop and implement personalized care plans in collaboration with physicians and multidisciplinary care teams
- Administer wound care, medication management, patient education, and ongoing disease monitoring
- Relay patient status updates and significant changes to the clinical team
Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience
- Strong OASIS documentation skills (required)
- Proficiency with HomeCare HomeBase (highly preferred)
- Reliable transportation and a valid drivers license
- Excellent time management; adept at maintaining a robust full-time visit schedule
