This full-time opportunity is ideal for RNs who are confident in clinical delivery, OASIS documentation accuracy, and schedule management. Youll work with a professional care team during a 13-week contract, delivering high-quality patient care directly in the home environment.
Schedule and Hours:
- Full-time, Monday through Friday
- On-call requirements likely
Key Responsibilities:
- Deliver professional in-home nursing care, including skilled interventions tailored to each patient
- Perform comprehensive patient assessments and ensure timely, accurate OASIS documentation
- Collaborate with physicians and care teams to implement individualized care plans
- Provide wound care, oversee medication management, and educate patients/families on disease monitoring and health strategies
- Promptly communicate status updates and changes in patient conditions to the clinical team
Qualifications & Experience:
- Current, active RN license
- Minimum of one year recent experience as a home health registered nurse
- Demonstrated proficiency with OASIS documentation (required)
- Experience with HomeCare HomeBase (strongly preferred)
- Able to consistently manage a structured, full-time schedule and complete daily visits
- Valid drivers license and reliable transportation
