Schedule/Hours:
- Flexible contract-based scheduling
Qualifications:
- Active Registered Nurse (RN) license
- Minimum of 3 months of home health experience utilizing OASIS
- Exceptional clinical skills in IV therapy, wound care, and medication management
- Proficient with electronic medical records (EMR); HCHB experience preferred
- Excellent communication and interpersonal skills
- Ability to work independently and manage scheduling effectively
- Valid drivers license and reliable transportation
Responsibilities:
- Deliver comprehensive nursing care and perform thorough assessments in home health settings
- Develop individualized care plans tailored to patient needs
- Conduct patient education for disease management, treatment, and prevention
- Collaborate effectively with physicians, therapists, and the broader healthcare team
- Document all patient interactions and care in EMR accurately and promptly
- Monitor and promptly report changes in patient conditions
- Maintain compliance with state and federal regulations related to home health
