Your next nursing opportunity starts here. A dedicated Registered Nurse (RN) with OASIS experience is needed for a 13-week, full-time home health contract. If you excel in clinical care, accurate documentation, and managing a structured schedule, this position lets you make a meaningful difference in patients lives every day.
Schedule:
- 13-week assignment, with the potential for extension
- Full-time hours, Monday through Friday
- On-call responsibilities may be required
Key Responsibilities:
- Deliver skilled nursing care and interventions in patient homes
- Complete accurate, timely OASIS documentation and assessments
- Collaborate with physicians and multidisciplinary care teams to develop individualized patient care plans
- Perform wound care, medication management, patient education, and disease process monitoring
- Clearly communicate patient status and any changes to the clinical team
Qualifications:
- Current, active RN license
- At least one year of recent home health nursing experience
- Demonstrated proficiency with OASIS
- Experience using HomeCare HomeBase is strongly preferred
- Capable of handling a structured daily patient visit schedule
- Must have reliable transportation and a valid drivers license
