Deliver quality care and make a meaningful impact as an experienced RN specializing in home health. This full-time, 13-week contract role offers an engaging field-based environment, where you'll work independently while supported by a collaborative multidisciplinary team.

Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM

As part of this position, you will:

• Complete comprehensive OASIS assessments and develop tailored care plans for each patient.
• Provide skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management in patients homes.
• Monitor patient condition, track progress, and communicate any significant changes to physicians and the care team.
• Educate patients and families about disease management, treatment plans, medications, and preventative techniques.
• Document all care and patient interactions using the Home Care Home Base (HCHB) system, ensuring accuracy and compliance.
• Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides for seamless care coordination.
• Attend case conferences, participate in ongoing training, and uphold regulatory and agency standards.

Requirements:

• Active RN License in good standing for the state of practice
• Minimum one year of recent field-based Home Health experience
• Proficient in Home Care Home Base (HCHB)
• Experience with OASIS assessments
• Reliable transportation, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
