Key Responsibilities:
- Conduct initial and routine patient assessments, including vital signs, medication management, and wound care.
- Develop and implement personalized care plans tailored to individual patient needs.
- Educate patients and families on disease management, treatment options, and preventive health measures.
- Collaborate with physicians, therapists, and healthcare professionals to coordinate comprehensive care.
- Accurately document all patient interactions and care activities in electronic medical records (EMR).
- Monitor patient conditions closely and communicate changes promptly to supervising physicians.
- Ensure compliance with all applicable state and federal home health care regulations.
Qualifications:
- Active Registered Nurse (RN) license.
- Minimum of three months home health experience utilizing OASIS.
- Experience with Homecare Homebase (HCHB) software is a significant advantage.
- Strong clinical skills including IV therapy, wound care, and medication administration.
- Proficiency in using electronic medical records (EMR) systems.
- Excellent communication and interpersonal abilities.
- Ability to work independently and manage time efficiently.
- Valid drivers license and reliable transportation.
