As a PACE Home Health RN, youll sharpen your nursing skills in a diverse, collaborative environmentworking alongside an interdisciplinary team to coordinate proactive, comprehensive care for seniors wishing to remain independent at home. Daily responsibilities build your expertise in patient assessment, care planning, and ongoing evaluation, while refining your communication and documentation capabilities across complex patient needs and family dynamics. Schedule/Hours

  • Full-time schedule with flexibility to address participant and interdisciplinary team needs

Qualifications & Desired Experience

  • Active, unencumbered RN license
  • CPR certification (current)
  • Valid drivers license, reliable transportation, proof of auto insurance
  • 12 years of recent hospital or acute care nursing experience
  • Home health or long-term care nursing experience preferred
  • Proficiency with EHR systems and Microsoft Office
  • Strong interpersonal and communication skills; able to navigate sensitive situations tactfully

Key Responsibilities

  • Assess participant and family needs, develop and oversee tailored care plans
  • Deliver direct nursing care and provide education, counseling, and preventive support
  • Coordinate care with a multidisciplinary team and ensure seamless communication/documentation
  • Continuously reassess care plans to adapt to changing participant needs
