This position is ideal for RNs confident in both clinical judgment and documentation, with a proven ability to work efficiently in patients homes.
Schedule:
- 13-week contract with potential for extension
- Full-time hours: Monday through Friday
- On-call requirements likely
Key Responsibilities:
- Deliver nursing care and skilled clinical interventions in patient homes
- Conduct comprehensive assessments and complete OASIS documentation thoroughly and promptly
- Collaborate with physicians and multidisciplinary care teams to develop and implement individualized care plans
- Perform wound care, medication management, patient education, and disease monitoring
- Communicate effectively with clinical team regarding patient status changes
Desired Qualifications:
- Active RN license
- At least one year of recent home health RN experience
- OASIS documentation experience is required
- Proficiency using HomeCare HomeBase strongly preferred
- Ability to maintain a structured daily schedule and meet visit requirements
- Reliable transportation and a valid drivers license
