A dynamic opportunity is available for a Registered Nurse with expertise in Home Health to support patients across Lexington and surrounding areas. This is a full-time, 13-week contract position focused on delivering high-quality, compassionate care in a field-based setting.
Schedule & Work Setting:
- Monday Friday, 8:00 AM 5:00 PM
- Field-based, providing care in patient homes
- Expectation of 30 points per week (typically 35 patient visits per day)
- Daily travel within assigned territory (mileage reimbursement provided)
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized patient care plans
- Provide skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management
- Monitor patient progress and proactively communicate changes in condition to physicians and the care team
- Educate patients and families regarding disease processes, treatment plans, medications, and preventive care
- Ensure accurate and timely documentation using Home Care Home Base (HCHB)
- Collaborate with interdisciplinary professionals such as physical and occupational therapists, social workers, and aides
- Participate in case conferences, staff meetings, and ongoing professional development
- Adhere to all regulatory requirements and agency standards for patient care and documentation
Required Qualifications:
- Active RN license in good standing for the state of practice
- At least 1 year of recent field Home Health experience
- Proficiency with Home Care Home Base (HCHB) and OASIS
- Reliable transportation, valid drivers license, and auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
