A dedicated Registered Nurse with strong OASIS experience is sought to fill a 13-week contract specializing in home health. Schedule:
- 13-week full-time contract (potential for extension)
- Monday through Friday
- On-call rotation likely
Key Responsibilities:
- Deliver skilled in-home nursing care and interventions to diverse patient populations
- Conduct comprehensive patient assessments and complete OASIS documentation with accuracy and timeliness
- Collaborate with physicians and interdisciplinary care teams to implement tailored care plans
- Perform essential care tasks, including wound care, medication administration, patient education, and chronic disease monitoring
- Clearly communicate any patient status changes or updates to the clinical team
Preferred Qualifications:
- Current, active RN license
- Minimum 1 year of recent home health RN experience
- OASIS documentation proficiency required
- Familiarity with HomeCare HomeBase (strongly preferred)
- Ability to manage daily visit schedules independently
- Reliable transportation and a valid drivers license
