This opportunity is for a Registered Nurse (RN) specializing in Home Health with OASIS experience, working a 13-week full-time contract in Lexington, KY. Key Responsibilities:

  • Deliver in-home skilled nursing care and interventions.
  • Perform comprehensive patient assessments, completing OASIS documentation accurately and promptly.
  • Collaborate closely with physicians and care teams to develop and implement individualized care plans.
  • Provide wound care, medication management, patient education, and monitor disease conditions.
  • Communicate patient updates and changes in condition effectively to the clinical team.

Qualifications:

  • Active Registered Nurse (RN) license.
  • Minimum of 1 year recent home health nursing experience.
  • Proven experience with OASIS documentation.
  • Proficiency with HomeCare HomeBase strongly preferred.
  • Ability to manage a full-time, structured schedule with daily patient visits.
  • Reliable transportation and valid drivers license.

Schedule and Location:

  • Full-time, Monday through Friday.
  • On-call duties are probable.
