This opportunity is for a Registered Nurse (RN) specializing in Home Health with OASIS experience, working a 13-week full-time contract in Lexington, KY. Ideal candidates are skilled in clinical care, accurate documentation, and managing structured schedules, ready to make a meaningful difference in patients lives.
Key Responsibilities:
- Deliver in-home skilled nursing care and interventions.
- Perform comprehensive patient assessments, completing OASIS documentation accurately and promptly.
- Collaborate closely with physicians and care teams to develop and implement individualized care plans.
- Provide wound care, medication management, patient education, and monitor disease conditions.
- Communicate patient updates and changes in condition effectively to the clinical team.
Qualifications:
- Active Registered Nurse (RN) license.
- Minimum of 1 year recent home health nursing experience.
- Proven experience with OASIS documentation.
- Proficiency with HomeCare HomeBase strongly preferred.
- Ability to manage a full-time, structured schedule with daily patient visits.
- Reliable transportation and valid drivers license.
Schedule and Location:
- Full-time, Monday through Friday.
- On-call duties are probable.
