Schedule:
- Full-time, Monday through Friday
- Possible on-call responsibilities
Key Responsibilities:
- Deliver skilled in-home nursing services while fostering patient comfort and safety
- Conduct thorough assessment visits with timely, accurate OASIS documentation
- Collaborate with physicians and a multidisciplinary team to implement personalized care plans
- Provide comprehensive wound care, medication management, patient education, and monitor chronic conditions
- Effectively communicate patient updates to the clinical team and promptly address any changes
Preferred Qualifications:
- Current, active RN license
- At least one year of recent experience as a home health RN
- Demonstrated proficiency with OASIS documentationa must-have for this role
- Experience using HomeCare HomeBase is strongly preferred
- Ability to fulfill a structured, full-time schedule managing daily patient visit targets
- Reliable transportation and a valid drivers license are required
