We are seeking a skilled Registered Nurse (RN) with proven OASIS experience for a 13-week full-time home health contract. Schedule & Hours:
- Full-time, Monday through Friday
- On-call responsibilities may be required

Key Responsibilities:
- Deliver comprehensive in-home nursing care and skilled interventions
- Perform patient assessments and submit accurate, timely OASIS documentation
- Collaborate with physicians and interdisciplinary care teams to devise and implement personalized care plans
- Manage wound care, administer medications, provide patient education, and monitor disease progression
- Maintain clear, effective communication regarding patient status and care updates
Qualifications:
- Active RN license
- Minimum of 1 year of recent home health RN experience
- Proficient in OASIS assessment and documentation
- Experience with HomeCare HomeBase strongly preferred
- Able to manage a structured full-time schedule and daily patient visits
- Reliable transportation and valid drivers license
