Your next nursing opportunity starts here. We are seeking a committed Registered Nurse with strong experience in home health and proficiency in OASIS documentation for a 13-week full-time contract. This role is perfect for an RN who is skilled at managing a structured schedule, has confidence in clinical skills, and values making meaningful connections with patients in their own homes.
Schedule:
- 13-week contract (potential to extend)
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Structured daily visit expectations
Key Responsibilities:
- Deliver in-home nursing care and skilled interventions
- Complete patient assessments with precise, timely OASIS documentation
- Partner with physicians and multidisciplinary teams to create and execute individualized care plans
- Perform wound care, medication management, disease monitoring, and patient education
- Accurately communicate changes in patient condition to the broader clinical team
Qualifications:
- Current, active RN license
- At least 1 year of recent home health RN experience
- Demonstrated competence with OASIS documentation (required)
- Experience with HomeCare HomeBase (strongly preferred)
- Comfortable managing a daily caseload and adapting to home health routines
- Valid drivers license and reliable transportation
