Schedule:
- 13-week contract, Monday through Friday
- Full-time, day shifts with probable on-call responsibilities
- Structured daily visit expectations
Key Responsibilities:
- Deliver professional in-home nursing care and skilled interventions
- Conduct thorough patient assessments and complete timely, accurate OASIS documentation
- Collaborate with physicians and multidisciplinary care teams to individualize patient plans
- Provide education, wound care, medication administration, and disease monitoring
- Maintain clear communication regarding patient status with the clinical team
What Were Looking For:
- Active RN license in good standing
- At least 1 year of recent RN home health experience
- Demonstrated OASIS competency (required)
- Experience with HomeCare HomeBase (strongly preferred)
- Ability to manage a structured full-time home health schedule and daily visits
- Reliable transportation and a valid drivers license
