Schedule:
- Full-time commitment, Monday through Friday
- On-call duties may be required
Key Responsibilities:
- Deliver skilled in-home nursing services, including wound care, medication management, disease monitoring, and patient education
- Conduct thorough patient assessments and complete OASIS documentation accurately and within set deadlines
- Collaborate closely with physicians and multidisciplinary care teams to tailor individualized care plans
- Communicate effectively with the clinical team regarding updates or changes in patient conditions
- Maintain accurate records and support optimal patient outcomes
Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience is required
- Proficient with OASIS documentation
- Strongly prefer proficiency with HomeCare HomeBase software
- Able to organize and execute daily patient visits according to a fixed schedule
- Must have reliable transportation and a valid drivers license
