A rewarding opportunity is available for an experienced Registered Nurse to provide essential care in a home health setting. This position is a full-time, 13-week contract with a Monday through Friday schedule from 8:00 AM to 5:00 PM. The RN will manage a caseload targeting 30 points per week, typically equating to 35 patient visits daily in a field-based role. Daily travel within the assigned territory is required, with mileage reimbursement provided.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM

• Field-based: patient homes
• Caseload: 30 points per week (35 visits daily)

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient progress, recognize changes, and communicate with the healthcare team and physicians as needed.
• Educate patients and families regarding disease management, medications, and preventive care.
• Ensure documentation is accurate and timely using Home Care Home Base (HCHB).
• Collaborate with interdisciplinary team members for coordinated patient care.
• Participate in case conferences, staff meetings, and ongoing education.
• Comply with regulatory and agency documentation and care standards.

Qualifications:

• Current and active RN license (in good standing)
• At least one year of recent Home Health nursing experience (field experience highly preferred)
• Experience with Home Care Home Base (HCHB) and OASIS is required
• Valid drivers license, reliable transportation, and auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
