Schedule/Hours:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM

As a field-based RN specializing in home health, your days will center around meaningful patient interactions within their own homes. With a weekly caseload expectation equivalent to about 35 patient visits per day, youll enjoy dynamic experiences and the chance to impact lives directly. Regular professional collaboration and continued education will further expand your clinical acumen and leadership skills.

Qualifications:

• Current RN License (state of practice, in good standing)
• Minimum 1 year recent home health nursing experience (field experience preferred)
• Proficiency with Home Care Home Base (HCHB)
• OASIS assessment experience
• Reliable transportation, valid drivers license, and auto insurance

Primary Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing care (wound care, medication administration, IV therapy, chronic disease management)
• Monitor patient progress; promptly communicate changes to the care team and physicians
• Educate patients and families on care plans and preventive strategies
• Maintain accurate, timely documentation in HCHB
• Collaborate with interdisciplinary team members to ensure coordinated care
• Attend staff meetings, case conferences, and participate in ongoing education
• Comply with all regulatory and agency standards

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
