Job Description

Step into a rewarding nursing opportunity providing high-quality care in a home health setting. This full-time contract position spans 13 weeks, focusing on field-based patient care and comprehensive case management. The ideal clinician will bring expertise in home health nursing and demonstrate strong assessment, documentation, and patient education skills.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform OASIS assessments and develop tailored care plans for each patient.
• Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management in patient homes.
• Monitor patient conditions, document progress, and communicate changes promptly to healthcare teams and physicians.
• Educate patients and families about medical conditions, medications, and preventative strategies.
• Accurately maintain clinical records using Home Care Home Base (HCHB) software.
• Collaborate with interdisciplinary professionalsincluding therapists, social workers, and home health aidesto ensure holistic, coordinated care.
• Attend case conferences, staff meetings, and engage in ongoing education to stay current with best practices.
• Uphold regulatory standards and agency requirements for home health documentation and care delivery.
• Daily travel required within a defined territory; mileage reimbursement provided.

Qualifications:

• Current, active RN license (in good standing) for the state of practice.
• At least 1 year of recent home health experience, ideally field-based.
• Strong proficiency in Home Care Home Base (HCHB) and completion of OASIS assessments.
• Must possess reliable personal transportation, valid drivers license, and active auto insurance.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
