Were seeking a skilled Registered Nurse (RN) with strong Home Health and OASIS experience to provide essential patient care on a full-time, 13-week contract. If you excel at clinical documentation, patient assessments, and thrive in a structured environment, this could be the ideal next step in your nursing journey.
Schedule:
- Full-time, Monday through Friday
- Daily visit expectations
- Potential for on-call responsibilities
Key Responsibilities:
- Deliver in-home nursing services, including skilled assessments and clinical interventions
- Complete OASIS documentation with accuracy and timeliness
- Collaborate effectively with physicians and interdisciplinary care teams to develop and deliver individualized treatment plans
- Provide wound care, manage medications, educate patients and families, and monitor for disease progression
- Maintain clear communication with the clinical team about patient updates and changes in condition
Qualifications:
- Active, unencumbered RN license
- At least one year of recent Home Health RN experience
- Demonstrated proficiency with OASIS documentation
- Familiarity with HomeCare HomeBase EMR (strongly preferred)
- Reliable transportation and a valid drivers license
- Strong organizational skills and the ability to manage a full-time home visit schedule
