Step into a vital field-based nursing role providing patient-centered care in home health. This full-time, 13-week contract offers a Monday to Friday schedule from 8:00 AM to 5:00 PM, with a caseload expectation of 30 points per weektypically resulting in 35 patient visits per day. Youll serve individuals in their homes within an assigned territory, supported by mileage reimbursement and a collaborative multidisciplinary team.

Essential Qualifications:

• Active Registered Nurse license in good standing for the state of practice
• At least 1 year of recent home health nursing experience (field experience highly regarded)
• Strong proficiency with Home Care Home Base (HCHB) electronic documentation
• Proven OASIS assessment and documentation skills
• Reliable transportation, valid drivers license, and current auto insurance

Key Responsibilities:

• Conduct comprehensive OASIS assessments, develop and manage individualized care plans
• Administer skilled nursing interventions, including wound care, medication management, IV therapy, and chronic disease support
• Regularly evaluate patient progress; communicate changes or concerns promptly to care teams and physicians
• Deliver education to patients and families regarding treatments, preventive care, and medication
• Diligently complete documentation and charting in HCHB
• Engage in collaborative care with team members across disciplines
• Attend scheduled case conferences, staff meetings, and ongoing education
• Uphold regulatory and organizational standards for documentation and care

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
