Schedule/Hours:
- Full-time, Monday through Friday
- On-call availability may be required
What Youll Do:
- Deliver skilled nursing care and interventions in patient residences
- Complete accurate and timely patient assessments utilizing OASIS documentation
- Coordinate individualized care plans in collaboration with physicians and the care team
- Manage wound care, medication administration, disease-specific education, and monitoring
- Keep the clinical team informed of patient updates and changes in condition
What Were Looking For:
- Active RN license
- Minimum of one year recent experience as a home health RN
- Required: proficient with OASIS documentation
- Strongly preferred: experience with HomeCare HomeBase
- Demonstrated ability to handle a structured, full-time patient schedule
- Dependable transportation and valid drivers license

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
