We are seeking a dedicated Registered Nurse with a background in home health care and a strong command of OASIS documentation for a full-time, 13-week contract assignment. Schedule:
- 13-week contract, full-time
- Monday through Friday
- Potential on-call responsibilities
- Structured daily visit expectations
Key Responsibilities:
- Deliver skilled nursing care and interventions in patients homes
- Complete thorough patient assessments and OASIS documentation accurately and within timelines
- Develop, implement, and revise individualized care plans alongside physicians and care teams
- Administer wound care, manage medications, provide patient/family education, and monitor disease progression
- Maintain clear communication regarding patient status and updates with the clinical team
Qualifications:
- Current, active RN license
- At least one year of recent experience as a home health RN
- Required proficiency with OASIS documentation
- Experience with HomeCare HomeBase is strongly preferred
- Strong organizational skills to manage a full caseload and daily patient visits
- Reliable transportation and a valid drivers license
