A dedicated home health Registered Nurse (RN) is needed for a dynamic, 13-week full-time contract. Candidates with OASIS experience and a passion for delivering patient-centered care in a home environment are strongly encouraged to apply. Schedule:
- Full-time, Monday through Friday
- On-call coverage may be required
- Daily structured patient visits
Key Responsibilities:
- Deliver skilled in-home nursing care, including wound care, medication administration, and disease management
- Complete thorough patient assessments and OASIS documentation promptly and accurately
- Collaborate closely with physicians and other care professionals to create and implement individualized care plans
- Educate patients and families on disease processes, self-care techniques, and medications
- Monitor and communicate changes in patient condition to the clinical team
Who You Are:
- Active, unencumbered RN license
- At least 1 year of recent home health RN experience
- Demonstrated OASIS documentation skills (required)
- Proficient with HomeCare HomeBase (strongly preferred)
- Excellent organizational skills and ability to manage a consistent, full-time visiting schedule
- Reliable transportation and valid driver's license
