A rewarding full-time 13-week contract is available for an experienced Registered Nurse specializing in Home Health. If you excel in clinical care, precise documentation, and enjoy creating positive impacts for patients in their own homes, this opportunity could be your ideal next step.
Schedule:
- Full-time, Monday through Friday
- On-call duties are probable
Key Responsibilities:
- Deliver in-home nursing care, skilled interventions, and patient education
- Perform comprehensive assessments and complete OASIS documentation accurately and promptly
- Collaborate with physicians and interdisciplinary care teams to provide individualized care plans
- Administer wound care, medication management, and chronic disease monitoring
- Communicate status updates and significant changes in patient conditions to the clinical team
Who We Are Looking For:
- Active RN license, in good standing
- At least 1 year of recent experience as a Home Health RN
- OASIS documentation expertise is required
- Proficiency with HomeCare HomeBase is strongly preferred
- Reliable organizational skills to handle structured daily visits
- Valid drivers license and dependable transportation
