Take your clinical expertise beyond the walls of a hospital by providing compassionate care directly in patients homes. We are seeking a Registered Nurse (RN) experienced in home health and OASIS to support patients during a full-time, 13-week contract. If you excel in detailed documentation, clinical assessments, and thrive in an independent work setting, consider this rewarding position.
Schedule:
- 13-week contract with the possibility of extension
- Monday through Friday, full-time
- On-call rotation may be included
- Daily visit goals set for consistency
Key Responsibilities:
- Deliver skilled nursing care during scheduled home visits
- Complete thorough patient assessments and timely OASIS documentation
- Collaborate with physicians and interdisciplinary teams to ensure high-quality, personalized care plans
- Provide wound care, administer medications, offer patient education, and monitor disease progression
- Communicate promptly with the clinical team regarding changes in patient status
Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience
- Demonstrated experience with OASIS documentation (required)
- Proficiency using HomeCare HomeBase (strongly preferred)
- Ability to manage a full-time, structured visit schedule
- Reliable vehicle and valid drivers license
