We are seeking a dedicated Registered Nurse (RN) with strong Home Health experience for a full-time, 13-week contract. If you are skilled in clinical care, documentation accuracy, and managing a consistent schedule, this position offers the chance to make a meaningful difference in patients lives.
Schedule & Hours:
- Full-time, Monday through Friday
- On-call responsibilities may be included
What to Expect:

Key Responsibilities:
- Deliver in-home nursing care and skilled interventions
- Complete patient assessments and ensure timely, accurate OASIS documentation
- Collaborate closely with physicians and multidisciplinary teams to develop and implement individualized care plans
- Perform wound care, medication management, patient education, and monitor disease progression
- Communicate changes in patient status to the clinical team promptly
Who You Are:
- Active Registered Nurse (RN) license
- Minimum of 1 year recent experience as a Home Health RN
- Proficient with OASIS documentation (required)
- Familiarity with HomeCare HomeBase is strongly preferred
- Able to maintain a structured, full-time workload with daily patient visits
- Possess reliable transportation and a valid drivers license
