Your next nursing opportunity starts here. We're seeking a dedicated Registered Nurse (RN) with OASIS experience to step into a 13-week full-time home health assignment. This role is ideal for RNs who excel in clinical care, thorough documentation, and thrive within a structured schedule, making a meaningful difference in patients lives every day.

Schedule:

• 13-week contract with the possibility of extension
• Full-time, Monday through Friday
• On-call coverage is probable

What youll do:

• Deliver skilled nursing care to patients in their homes, ensuring professional standards are met
• Complete comprehensive patient assessments and OASIS documentation with accuracy and timeliness
• Support care coordination by collaborating with physicians and interdisciplinary teams to implement individualized care plans
• Perform tasks including wound care, medication management, patient/family education, and ongoing disease monitoring
• Communicate patient status, changes, and updates clearly to the broader clinical team

Qualifications:

• Active and unrestricted RN license
• Minimum of 1 year recent RN home health experience
• Strong proficiency with OASIS documentation (required)
• Experience with HomeCare HomeBase electronic charting (highly preferred)
• Valid drivers license and reliable transportation
• Able to manage a structured daily schedule with multiple home visits
