Your next nursing adventure begins with this rewarding full-time opportunity for an experienced Registered Nurse seeking to provide exceptional care in a home health setting. This 13-week contract offers the chance to work with a dedicated team, utilizing your expertise to make a meaningful impact for patients in their own homes.
Schedule:
- Full-time, Monday through Friday
- Structured daily visit expectations
- On-call responsibilities are likely
Key Responsibilities:
- Deliver skilled in-home nursing care, including wound care, medication management, disease monitoring, and patient education
- Perform comprehensive patient assessments with accurate and timely OASIS documentation
- Collaborate with physicians and multidisciplinary care teams to develop and update individualized patient care plans
- Communicate changes in patient status and coordinate with the clinical team
- Maintain organized records of patient visits and interventions
Qualifications:
- Active RN license
- Minimum of 1 year recent experience in home health nursing
- Required: Proficiency with OASIS documentation
- Strongly preferred: Experience with HomeCare HomeBase
- Reliable transportation and valid drivers license
- Ability to independently manage a full caseload with high standards of clinical and documentation accuracy
