Your next nursing opportunity starts here. We are seeking a committed Registered Nurse (RN) with proven OASIS experience for a 13-week full-time home health contract. This role requires confidence in delivering skilled patient care, attention to documentation, and the ability to thrive within a structured schedulean ideal fit for RNs passionate about making a meaningful impact through home health services.
Schedule:
- 13-week contract with strong potential for extension
- Full-time, Monday through Friday
- On-call responsibilities probable
- Set daily visit expectations
Key Responsibilities:
- Deliver in-home nursing care and skilled interventions
- Complete thorough patient assessments and OASIS documentation accurately and promptly
- Collaborate with physicians and interdisciplinary care teams to develop and implement individualized patient care plans
- Administer wound care, manage medications, provide disease monitoring, and foster patient education
- Consistently communicate patient status updates to the clinical team
Desired Qualifications:
- Active RN license (required)
- At least 1 year of recent RN experience in home health settings
- Demonstrated proficiency with OASIS documentation
- Familiarity with HomeCare HomeBase strongly preferred
- Dependable transportation and a valid drivers license
- Organized, self-motivated, and comfortable managing a full-time clinical visit schedule
