Schedule:
- 13-week contract, with potential to extend
- Full-time, Monday through Friday
- On-call hours may be required
- Structured daily visit expectations
Qualifications:
- Active RN license
- At least 1 year of recent RN experience specifically in home health
- Proficiency with OASIS documentation is required
- Strongly prefer experience with HomeCare HomeBase
- Reliable transportation and valid drivers license
- Ability to effectively manage a full-time, structured schedule
- Excellent clinical, communication, and documentation skills
Responsibilities:
- Deliver in-home skilled nursing care and interventions
- Conduct comprehensive patient assessments and ensure timely OASIS documentation
- Collaborate with physicians and interdisciplinary care teams to provide personalized care plans
- Administer wound care, medication management, disease monitoring, and patient education
- Communicate patient updates and changes in condition clearly with the clinical team
