Job Description
Discover an exciting opportunity for an experienced Registered Nurse with a passion for home health care. This full-time, 13-week contract is designed for an RN who thrives in independent clinical settings, is meticulous with documentation, and enjoys working one-on-one with patients in their homes.
Schedule:
- Full-time, Monday through Friday
- On-call rotations may be required
- Daily visit quotas for patient care
Key Responsibilities:
- Deliver comprehensive in-home nursing care and skilled interventions
- Accurately conduct assessments and complete OASIS documentation within required timelines
- Create, implement, and modify individualized care plans in collaboration with physicians and interdisciplinary teams
- Perform wound care, oversee medication administration, and provide patient and caregiver education
- Monitor disease progression and promptly communicate any changes to the clinical team
Required Qualifications:
- Active and unrestricted RN license
- Minimum of one year of recent home health experience as a registered nurse
- Demonstrated proficiency with OASIS documentation
- Experience with HomeCare HomeBase is strongly preferred
- Exceptional organizational skills with the ability to effectively manage a structured full-time schedule
- Reliable transportation and a valid drivers license

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
